Not Compliant With Your Health History?

Thought you were done with your Health History Form, but then you received a letter from Gannett stating that you are not yet in compliance?

  1. Please access your immunization record by clicking on the myGannett button on the right side of this page.
  2. Once you see the immunizations for which you are not compliant, consider the questions below.

Tetanus

  • Is the tetanus you provided with in 10 years? If not you will need to update.

Measles/Mumps/Rubella

Two doses of live vaccine must be administered no more than 4 days before your 1st birthday. The two doses MUST be a minimum of 28 days apart.

  • Have you had 2 doses of MMR (Measles, Mumps, Rubella)?
  • Are your MMR doses on or after one year of age?
  • Are your MMR doses at least 28 days apart?
  • If you have had the disease did you provide us with at least the year you had it?
  • Have you had titers done?  Did you supply us with authenticate documents of the result?

If any of these scenarios apply to you you may need additional doses.

Tuberculin Skin Test (PPD, Mantoux)

International students from countries with a high incidence of TB must have a US based tuberculin screening. If you can not have one prior to entry, then you must do this at Gannett Health Services upon your arrival at Cornell.  If you wait to have it at Cornell as long as you have the test with in 3 weeks of your arrival, you will not be assessed any penalties.

Chest X-ray

Your chest X-ray must be U.S.- based. Only necessary if you have a history of a positive tuberculin skin test.

  • Was your X-ray completed in the US?

Meningococcal

If you have had the vaccine within 5 years, we need proof of immunity. If you choose NOT to have the vaccine you will need to sign a waiver. If you are under 18 years of age you must have your parent/guardian sign the waiver.  

  • If you provided a vaccine date is it within 5 years?  
  • Are you choosing to waive the meningococcal vaccine? If so, please sign? If you under 18 years of age and choosing to waive, did your parent/guardian sign?

Proof of immunity

Written documentation you submit as proof of immunity must be from:

  • a physician or health care provider or
  • an immunization record from a previously attended school.
