Q: Where do I find my immunization records?
A: Check for your immunization records at any school you have attended or with your doctor’s office.
Q: What if I don't have access to my immunization records?
A: If you cannot locate your immunization records, you will need to receive the required vaccines again at Gannett Health Services (fees may be assessed). If you are certain you have been vaccinated for measles, mumps and rubella, you can ask a clinician to do a titer (blood test) to prove your immunity.
Q: Where is the online health history form located?
A: Go to http://newstudents.cornell.edu. Click on your enrollment status: first year, transfer, or graduate/professional. Select "To Do List." Under forms to fill out, select "Health History & Immunization Compliance," then select "Health History Form." Next, select "NetID login" and login using your NetID and password. Once you have logged in you will be at the start of the health history form. Please remember to read all instructions carefully.
Q: How can I confirm receipt of my health history form and/or compliance status?
A: Go to http://newstudents.cornell.edu and log in. Click on your enrollment status: first year, transfer, or graduate/professional. Select "To Do List." This section will assist you with information related to the status of your health history form and/or immunization compliance. In order to see your most up to date status, check to be sure you have logged in.
Q: How do I correct a mistake I have made on the online portion of the health history form?
A: Changes can be made in ink to the printed copy. Once we receive the form we will make the necessary changes electronically.
Q: My form is going to be late. Is this a problem?
A: We ask that you mail the form as close to the deadline as possible. If that is not possible then mail it when you can. Allow at least 15 business days for processing. Our goal is to process all forms prior to the $100 penalty date August 10, 2012 (January 11, 2013 if entering Spring semester 2013). Forms are processed in the order received. If your form does not get processed prior to the penalty deadline you may be assessed the $100 penalty.
Q: Do I have to have a physical exam?
A: It is recommended that ALL students complete a physical exam. However, any student planning to participate in a NCAA sport must have a physical exam performed after March 1, 2012.
Q: I have been a student at Cornell in the past; do I still need to complete the health history form?
A: Yes. However, it is necessary to complete only parts 1 and 5. Complete health history form online, submit, print the pdf, sign part 5, and mail to the address provided. Also, check to be sure your immunizations are up to date by signing into myGannett (click on upper right blue tab).
Q: Does my doctor/primary care provider have to sign the health history form?
A: If you are able to provide authentic documentation of immunizations or a completed physical, your form does not require a doctor's signature. If you are not able to submit authentic documentation for the immunization section and/or the physical sections, it is mandatory that you have your doctor or primary care provider sign part 4 of your form. Forms will not be processed without authentic documentation or appropriate signatures.
Q: What are the immunization requirements?
A: The requirements for ALL new entering students are:
- TETANUS: Must be within last ten years.
- MEASLES/MUMPS/RUBELLA: TWO doses of live vaccine administered no more than 4 days before the 1st birthday (the two doses MUST be a minimum of 28 days apart). You may also show proof of disease for measles, or mumps (we do not accept proof of disease for Rubella) OR complete a titer (blood test) to prove your immunity.
- MENINGOCOCCAL: Vaccine within last 5 years. However, if you have not had the vaccine you may choose to waive the requirement. To waive, select the third option on the health history form (part 3 -- immunization section) under "Meningococcal" and sign the printed pdf form. If you are under the age of 18 your parent/guardian must sign.
- TUBERCULIN SCREENING: All TB screening for high risk international students must be completed at Gannett Health Services shortly after your arrival. TB screenings completed internationally prior to arrival will not be accepted.
Q: What can I do if I'm unable to complete the immunization requirements prior to arriving at Cornell?
A: Students must be vaccinated upon arrival. Immunizations can be completed at Gannett; however, fees may be assessed for vaccine services. For more information about the required vaccines and fees, call Gannett at 607 255-6951.
Q: How do I disclose, obtain or discuss my health information (or record)?
A: To obtain, disclose or discuss your patient health information from Gannett Health Services you will first need to complete a form entitled Authorization for Release of Health Records. If you have any questions please contact our office at 607 255-4082.
