Medical Clearance Procedure

In order to participate in intercollegiate athletics at Cornell, you must receive a formal medical clearance from Sports Medicine each year. Your coach will provide you with details.

First year student athletes

Before coming to Cornell, you must have a physical examination from your health care provider. (See Part IV of the Health History Form sent to you with Cornell Bound). You must activate your Cornell network ID and be listed on the on-line compliance roster in order to be considered for sports clearance. If you are not on the roster, please contact your coach. You will be asked to complete the Sports Clearance Form(pdf). When you come to campus in the fall, you will report to Gannett with your team at the scheduled time for your sports clearance.

Returning athletes

Your coach will instruct you about completing the online Sports Clearance Form, through which you will provide complete and up-to-date information about your health. When you return to campus in the fall, you will report to Gannett with your team at the scheduled time for your sports clearance.
